MESHPLASTY - NEW METHOD BY SUNRISE SAVES MOTHER AND CHILD
The case was with regards to a 36-year-old lady, from Tamil Nadu who was suffering from Chronic Infertility problem for last 8 years. She was suffering from Adenomyosis, which destroys the uterus and is considered to be a very serious health condition. She had multiple surgeries and cycles of IVF, and none of them were successful. Adenomyosis is a condition where the uterus becomes enlarged and comes with severe pain and bleeding during menstrual cycles. She was referred to Dr. Hafeez Rahman, Consultant Gynaecologist & Laparoscopic Surgeon at Sunrise Hospital, Kochi, by a Doctor in Chennai, 2 years back in 2021, who had the belief that Dr. Hafeez can fix this health condition. A year back, the lady was admitted in Sunrise hospital in Kochi location and had undergone a laparoscopic adenomyomectomy surgery, which was successful. Immediately after the surgery in 8 months, she underwent a successful IVF treatment. Baby had a normal growth in her uterus for 16 weeks and both, were in a good health condition. But she suddenly developed a severe bleeding condition and consulted a doctor in Tamil Nadu, who took an MRI, which showed that one part of the uterus was thinned out to less than 1 cm, and it was about to rupture with the baby inside. Not only that, the placenta was adherent to that part, and she was told the only option was to remove the uterus with the baby and, was also had advised that failure to do so will be life threatening to both, mother and Baby. This condition is a known fact with the present medical history and knowledge based study. The lady had gone to multiple doctors in the state of Tamil Nadu, to make sure if there are any other options available, but unfortunately everyone had the same opinion.
She was again referred to Dr. Hafeez by the doctors in Tamil Nadu, for fact that she did not want to lose the baby, and they were confident that Dr. Hafeez will surely come up with a positive treatment result. But the travel back to Kochi was a very dangerous step as the uterus was in a condition to rupture any time which was life threatening to the mother and the Baby. She was admitted again to Sunrise Hospital under Dr. Hafeez’s treatment, and he suggested a new method of MESHPLASTY of the uterus, which might save the uterus, mother and the Baby. Patient was very confident with the assurance given by Dr. Hafeez, and she had undergone the procedure the same day itself. The procedure was very successful one and the patient was completely aware that the same was an experimental method which the medical team had tried.
The hospital somehow wanted to carry on the pregnancy up to 24 weeks as the NICU at hospital was well equipped to deal with such complicated premature babies. To everyone’s surprise, starting 24th week, they could see that the mesh was supporting the uterus, and the team decided to continue with the pregnancy. It was a bonus that the baby was surviving well inside the uterus. At 28th week, against all the medical knowledge that’s so far available, the mother and Baby was found absolutely in good medical condition. Even Dr. Hafeez was not expecting this much success, and hospital decided to take the risk and continue with the pregnancy. Patient was admitted in the hospital for 6 months and the entire medical team was ready, anytime to conduct any surgical procedure to take the baby out alive, in case needed.
During 36th week, all her relatives staying at the hospital got affected with Covid and patient also showing symptoms of fever. But she was found Covid negative after the test was taken. Dr Hafeez felt it was a risk for her to continue with the pregnancy and took the major decision to do Cesarean, which was successfully done on 29th March, 2023.A healthy baby was delivered through a Cesarean. Patient was very much concerned that if her uterus was removed, she might end up losing her life. Dr Hafeez gave her full assurance on this procedure, and a thin part of the uterus was excised, and the uterus was saved. The mother, baby and uterus was in a very much safe condition, and she was even assured that she could have another baby. Throughout the pregnancy and other procedures, the patient was looked after by Dr. SONIA FARHAN and Dr. AYESHA TANIYA. In addition to them the medical team who took care of the mother and the baby comprised of Dr. ABY KOSHY, Dr. SHAJI P.G, Dr. ANIL V AND Dr. ABEY MATHEW.
The entire procedure and outcome was indeed a great Miracle and was being exercised for the FIRST TIME IN THE WORLD MEDICAL HISTORY. The fact that a rupturing uterus was repaired by a unique procedure and that too WITH THE LIVE MOVING BABY INSIDE. This was the second patient undergoing similar procedure. The first patient had got it done at 14 weeks but had gone back to Tamil Nadu and had a premature delivery at 23rd week, but over there, they could not save the baby. It is indeed a proud moment that this technique was designed at Sunrise Hospital, Kochi by Dr. Hafeez Rahman, 5 years ago, but that was initially for large Adenomyosis to prevent rupture of the uterus, and it was successful at that time. But what’s unique this time was that this improvised procedure was exercised SUCCESFULLY, FIRST TIME IN THE WORLD ON A PREGNANT UTERUS.
This is indeed a historical breakthrough in the treatment for similar patients. Even though Rupture uterus is not a very common condition, it does occur and almost 80 percent of the time, mother will lose the baby. The incidence of fibroid uterus is increasing in numbers and the chances of rupture uterus, will still increase to higher numbers.
